Becoming a Patient
How do I become a patient?

To become a patient, fill out this waitlist form. Dr. Yurkish is currently accepting new patients, and there is a waitlist for care. The estimated wait time is 3-6 months.
Is Dr. Yurkish accepting new patients?

Dr. Kevin Yurkish is currently accepting new patients on a waitlist.

There is no waitlist for massage therapy! Select “New Patient Exam - Massage Referral” to be seen by Dr Yurkish for a referral for massage therapy.

I am already a patient of Dr. Yurkish, which type of appointment should I schedule? ​

If you have been seen in the last 3 years, but have a new complaint please select “Existing Patient Exam”

If you are under active care, please select “Follow-up Chiropractic Treatment” and note that there are both 25- and 55- minute options.

If it has been 3+ years since you were seen by Dr. Yurkish, you will need to be place on the waitlist for a new patient appointment.

Do I have to travel to the clinic for my appointment?

For your convenience, Dr. Yurkish is offering some of his services virtually or in the comfort of your own home/space. If you have a follow-up health counseling appointment, you may request a virtual appointment. If you prefer to have treatment at home, at work, or at an event, Dr. Yurkish is able to make House Calls. Please consider this option if you are unable to climb a flight of stairs, as his office is on the second floor without an elevator.
How do I prepare for my appointment?

Please fill out the medical intake forms, consent forms and submit your insurance information if applicable. During your visit, please wear clothing that allows you to move freely and remove any jewelry that may interfere with manual therapy. If you have long hair, please tie it up.

Do you work with children

Dr. Yurkish is a board certified chiropractic physician. His education includes training in pediatric care and he provides care to kids 12 yrs and older. He is excited to help improve the health of our next generation by instilling healthy habits and sustainable approaches to healthcare at a young age.
Do you work with people who are pregnant and postpartum?

Dr. Yurkish especially loves being a part of his patient's parenthood journey. He has extensive training in soft tissue treatment, spinal/pelvic alignment and pilates based rehab to help birthing persons prepare for and recover from childbirth. He works in conjunction with pelvic floor therapists, chiropractic specialists and medical providers to ensure the highest quality care for his patients.

Do you offer massage therapy?

Can massage be billed to insurance?

If you are interested in billing massage to your insurance, you will need to establish care with Dr. Yurkish first. Then, he will activate the referral for massage to be billed to insurance.

Can I bill massage to insurance without seeing a chiropractor?

You have to be under active care by Dr. Yurkish to be eligible to bill your massage to insurance.

What are the access concerns?

The clinic is located on the second floor of a historic building. Access to the clinic is atop a flight of stairs. There is no lift/elevator available, if this presents an issue please see House Calls for appropriate services.
Rates, Policies & Insurance
Do you take my insurance?
-
Dr Yurkish accepts insurance!

He is currently in network with the following plans: Regence/BCBS, Providence, PacificSource, and MODA.

While he is out-of-network with all other health insurances, there are many insurers he works with who offer out-of-network chiropractic benefits, such as: Aetna, United Healthcare.

He is out of network and unable to submit claims with: Kaiser, Cigna, OHP/Tricare
I don't have insurance, do you have a cash rate?
-
Yes, the cash rate is based on Oregon Worker's Compensation Physician's Fee Schedule. Medicare rules allow physicians to offer a 20% discount on services paid in cash. Each treatment might not be the same price depending on which services are offered.

You are also able to pay for treatment using FSA or HSA cards!

Dr Yurkish offers a financial hardship rate as well, if these prices are outside of your means please reach out to him via email to discuss this option.

Do you accept Motor Vehicle Accident claims?

You must have an active claim number prior to your first appointment.

Oregon is a “no-fault” state when it comes to car accidents, and all auto insurance policies carry personal injury protection (PIP) coverage. This means that regardless of who was at fault, your medical treatment will be covered by your auto policy's PIP coverage, usually at no cost to you.
What is the cancellation policy?

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a gap in the schedule that could have been filled by another patient. As such, Dr Yurkish requires 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee.

Generally, the cancellation fee is $75. However, frequent late cancellations or missed appointments may result in an increase to cover cost of the appointment, payable prior to any further appointment being made.
